Rehab at Home

Do you require some help to improve your fitness or prevent you falling and regain more independence? We can visit wherever you live, we visit our clients in care homes as well as your own home.

Assessment

At Ann Mason Care we are a progressive forward-thinking company – our aim to is enable as much independence for our clients as possible. We recognise the benefits, both psychologically and physically, for individuals to live as independently as possible.

Building on our “re-abled care” ethos we have developed a bespoke rehabilitation service and following the completion of a successful pilot scheme we are now offering this as one of our key services.

Our team of qualified Nurses, Physiotherapists and Rehab assistants all understand that Rehabilitation is the key to helping individuals achieve as active, healthy and independent life as possible. Our clients vary from those who need help to manage a long-term condition to those who have de-conditioned following a prolonged stay in hospital (due to injury or illness).

Programme goals

Our Physiotherapists will carry out a full assessment and set some goals agreed with you. They will then design and write up a bespoke rehab program to achieve these goals.

At the first set up session, you will be introduced to your rehab assistant who will then visit you, a pre agreed number of times a week, (from one to 5 times per week) and help and support you to carry out your rehabilitation programme. Your rehab assistant will help encourage, build your confidence, and help you to enjoy the recovery process.

Your road to recovery will be overseen by the physiotherapy team, who regularly meet with the rehab staff for feedback. The physiotherapist will visit you periodically to review your progress to ensure you are progressing to meet your ultimate goals.
