I would like to express my thanks to Ann Mason Care (AMC), especially Ann, Jan and the team who have supported me over the past 12 months
I first met Jan for an assessment whilst I was in hospital and she helped me plan my reablement programme and we agreed a live in carer would be the best to ensure I was supported as initially my needs were more complex.
Jan ensured all my carers had the required skills and were scheduled for when I returned home from hospital. Close liaison with the hospital discharge team and visiting specialist took place and my discharge went ahead smoothly.
After 3 months my family met with Jan to discuss my improving health. Following a careful assessment, a decision was taken to reduce my care needs to 3 visits a day
When I needed an emergency hospital procedure Jan again responded and liaised with my family to reinstate Live in care allowing me to recover minimising complications with my recovery
After 9 months of face to face care visits I was enthusiastic to participate in new technology enabled care (TEC) with scheduled remote video calls. I enjoyed these calls and we chatted to establish that all essential aspects of my self-medication and diet were progressing satisfactorily.
I am now happily living independently and still enjoy my weekly TEC calls with the great team at Ann Mason Care
